New Patient Appointments
If you would like to make an appointment at the McKinney office, please call (972) 542-5980. Leave a detailed message and a daytime number. We will call you to schedule the appointment. We are now scheduling March appointments.

Medication Refills
The Shreveport office closed in May of 2014. Dr. Bellah is no longer providing refills for patients from the Shreveport office. If you are a former patient and need medication refills, please contact your new psychiatrist or your primary care physician. If you have a medication emergency, please go to the local emergency room. If you have questions please call (972) 542-5980 for assistance.

If you are an established patient in the McKinney office, please have your pharmacy fax a medication refill request to (972)-542-5490.

Medical Records
If you are transferring to a new physician for services, we can send your medical records directly to your new physician's office. Please have the physician's office contact us at the McKinney office. Once we receive a release of information, your written permission, we will fax a copy of your records directly to your new physician's office.
